Acknowledgement and Assumption of Risk

I, the undersigned parent or legal guardian, understand that participation in activities at Playtime in the Plains involves certain inherent risks, including but not limited to falls, collisions, and other risks associated with indoor play equipment. I acknowledge that my child’s participation is voluntary, and I assume all risks associated with participation.

Waiver and Release

In consideration of my child being allowed to participate in activities at Playtime in the Plains, I, on behalf of myself, my child, and our respective heirs, executors, and administrators, release and discharge Playtime in the Plains its owners, employees, and a􀀁liates from any and all claims, liabilities, demands, or causes of action arising from or related to participation in activities at the facility, including but not limited to injury, illness, or property damage.

Supervision and Conduct

I understand that I am responsible for supervising my child at all times while at Playtime in the Plains. I agree to follow all posted rules and instructions given by sta􀀂 members. I acknowledge that failure to adhere to the facility’s rules may result in removal from the premises without refund.

Photo/Video Release

I grant permission for Playtime in the Plains to use photographs or videos of my child for promotional purposes, including social media, website, and printed materials. (Please check one)
